2025-2026 Pre-Participation Form & Sports Physical
When planning to participate in activities, please be aware that three mandatory fees must be paid to participate in the student's desired activity:
Activity Card Fee: $30 (one-time fee)
Participation Fee: $75
Specific Activity Fee: Varies based on activity
If a student plans to participate in multiple activities throughout the year, please note that a participation fee is required for each activity.
